Insurance Information
There are many ways to use your insurance benefits for lactation care! Check out our options below.
The Lactation Network is a company that helps families utilize their out of network benefits. See if you qualify for coverage here, and then come back to this page to schedule. Do not fill out this form if you have Aetna.
We are in-network providers with Aetna.
We are in-network providers with Anthem. This applies to occupational therapy services only. Anthem does not consider lactation to be a covered service in KY unless rendered by an MD or NP. See above for more information on how to utilize your out of network benefits using The Lactation Network.
Private Pay Information
For patients not using insurance, appointment pricing is as follows:
- Initial Office Visit Cost: $200 (Follow-up: $150)
- Initial Telehealth Visit Cost: $200 (Follow-up: $150)
- Initial Home Visit Cost: $300 (Follow-up: $275)
Check your out of network benefits – you may be eligible for reimbursement or application of visit costs to your deductible.
